No-Cost Vaccination Clinic at Hamilton

Effective November 27, 2023, your student MAY NOT ATTEND SCHOOL until an up-to-date immunization record, in-process plan or an exemption is on file at Hamilton Middle School.

  • You can make a FREE appointment with Denver Health School-Based Health Clinics by calling 303-602-8958
  • Complete a consent form for the Denver Health Immunization Clinic during school hours on 11/16/2023. We need immunization clinic consent returned by 11/10/23. 
  • Complete an immunization exemption form through your primary care provider and return to Hamilton’s front office.
