In-School Immunization Program Clinic Reminder
Hamilton Middle School is partnering with the Immunization and Travel Clinic at Denver Health
to offer in-school immunizations to students at Denver Public Schools in order to ensure
students have a healthy school year.
Next in-school immunization clinic date: October 19th.
About the In-School Immunization Program:
- One signed consent form allows your child to participate in any In-School Immunization
- Program clinic during the current school year.
- If you have completed and returned a consent form, your child will receive any vaccines
- that they are eligible for on this date, including COVID-19 and flu, unless any specific
- vaccines were declined on the consent form. Please make sure the school nurse has
- your child’s most up-to-date immunization record.
- Please make sure your child has eaten breakfast before coming to school on the clinic
- date.
- If you have signed a consent form, but no longer want to participate in the
- immunization program, please contact the school nurse Regan Thornburg (720) 423-9509 or Denver’s In-School

Hamilton Middle School esta colaborando con la Clinica De Immunizacion y Viaje en Denver
Health para offrecer immunizaciones a sus hijos que van a Denver Public Schools mientras
estan en la escuela. Assemos esto para esegurarnos un año escolar saludable para todos.

- Un formularion llenado y entregado a tiempo le daria permiso a su hijo participar en el
- programma de immunizacion durante este año escolar
- Esi usted a yenado un formulario su hijo recibira TODAS las vacunas requeridas para
- su salud y el Flu con la vacuna del Covid-19. Esi hay vacunas en particular que no
- quiere que su hijo reciba por favor indica cuales en el formulario
- Por favor aseguranse que su hijo come su desayuno antes de venir a la escuela el dia
- de la immunizacion
- Esi usted a llenado un formulario y lo a entregado pero no quiere que su hijo
- participle en el programa de immunizacion por favor contacta la enfermera de la
